I am still having problems lining up the information on this form in the lower right hand area. This is the closest I have gotten..I have the top set at 700, Bottom set at 700, Left set at 600 and right at Zero. It is still to high.

I would suggest that you just try tweaking the top margin a bit more, perhaps setting it to 850 and trying that. I would also recommend setting the bottom margin back to its default setting of 576 – or even setting it lower to something like 500 – so that bottom “Place and Date” doesn’t get sent to a second page.
